Police Applicant Screening Service (PASS) Process
The PASS process is a unified pre-employment screening process operated by the 14 law enforcement agencies of Pinellas County, Florida. It makes a single application simultaneously available to all these agencies to save time and effort for both you and the agencies.

Personal History Form - Note: Please fill out this form and BRING it to your Administrative Interview. DO NOT MAIL IT TO THE PASS OFFICE.
Instructions
- The Personal History Form (PHF) must be completed by the applicant.
- Use this form. (Make a copy if you want to create a draft first.) Hand print answers in ink using UPPERCASE LETTERS, or type using a standard font (similar to these instructions).
- Read all questions carefully before you start filling in the form.
- Answer all questions fully and accurately. All information is subject to verification by PASS Investigators and Polygraph Examination.
- Leave no spaces blank. If a question does not apply, mark "N/A" in the space.
- Provide all information. If any answer needs more space, use the back of the sheet.
- Complete mailing addresses for residences, employers and references are mandatory and must include house number, box number or apartment number (if applicable), street name, city, state and zip code (or foreign country and mailing code).
- Many questions ask for the "county" (the political sub-division of a state) in which you lived or an event occurred. Do not mistake this for "country" (i.e. a nation).
- Telephone numbers must include area code.
- These forms must be sworn and notarized before a representative of PASS.
- Do not sign or date the signature pages until told to do so at the PASS office.
WARNING Any misrepresentation, falsification, omission or concealment of a material fact will subject the applicant to disqualification and may subject the applicant to discharge if discovered subsequent to employment.
